Meet Lydia
Hello!Lydia received her Bachelor of Arts in Psychology with an emphasis in Clinical Psychology from Brigham Young University. She brings experience supporting individuals in behavioral and mental health settings, where she has helped people identify treatment goals, develop healthy coping skills and feel more confident participating in their care.
Throughout her work, Lydia has developed a compassionate and respectful approach to supporting people through difficult or overwhelming circumstances. She is experienced in listening carefully, building genuine connections and working alongside health-care professionals to help coordinate effective, patient-centred support.
As Cardston Clinic’s Community Health Navigator, Lydia works with patients to identify their needs, set realistic health goals and connect with helpful clinic and community resources. She is passionate about helping people better understand their options and take practical steps toward improved health and well-being.

Frequently miss appointments or have difficulty following a care plan
Would benefit from additional support and accountability
What to Expect
Lydia will take the time to learn about your needs, concerns and priorities. Together, you can identify your goals, explore available resources and create a practical plan that works for you.
Support may include:
An initial meeting to discuss your needs
Setting goals and creating an action plan
Connecting you with appropriate programs and resources
Regular follow-up and encouragement
Helping you prepare for or understand the next steps in your care
The Community Health navigator works alongside your physician and clinic care team to provide coordinated, patient-centred support
The Community Health Navigator does NOT provide:
Medical diagnoses or treatment
Personal care or housekeeping services
Crisis intervention
Transportation to appointments
Referrals to medical specialists - these continue to be completed by your physician
For urgent medical concerns or mental health crisis, patients should contact the appropriate emergency or crisis services.
